ABSTRACT

A boy, aged 17 years, was admitted again due to abdominal pain, diarrhea, and eosinophilia for 3 years, which worsened for 3 days. Three years ago, the boy suffered from abdominal pain and diarrhea after eating yogurt; color Doppler ultrasound showed a large amount of peritoneal effusion, and routine blood test, bone marrow cell morphology, and ascites histological examination showed a large number of eosinophils. Three days ago, he was admitted again due to abdominal pain and diarrhea. The gastrointestinal endoscopy showed eosinophil infiltration in the angle of stomach. The boy was diagnosed with eosinophilic gastrointestinal disease (eosinophilic gastroenteritis). He was improved after the treatment with glucocorticoids and dietary avoidance, and no recurrence was observed during the one-year follow-up. It is concluded that for children who attend the hospital due to gastrointestinal symptoms such as abdominal pain and diarrhea, if there is an increase in peripheral blood eosinophils, it is necessary to consider the possibility of eosinophilic gastrointestinal disease, and eosinophil infiltration and abnormal eosinophil count in gastrointestinal tissue based on endoscopic biopsy may be the key to diagnosis.

ABSTRACT

Objective To establish a modified dual liver transplantation rat model. Methods Based on the classic donor Y-shaped double iliac vein recanalization of bilateral liver grafts and portal vein and bile duct of the recipients, the dual liver transplantation rat model was modified by increasing the rat body mass, increasing the right lower lobe of the right graft, appropriate bile duct length, trimming Y-shaped blood vessels, and "triangular" anastomosis. The operation time, cold ischemia time, warm ischemia time and anhepatic phase of dual liver transplantation were recorded. The incidence of postoperative complications of the recipients was observed. The survival rates of the recipients at postoperative 7 and 30 d were calculated. Results The operation time of dual liver transplantation in rat was (114±7) min, the cold ischemia time was (36±3) min, the warm ischemia time was (9.7±1.6) min, and the anhepatic phase was (19.9±2.2) min, respectively. The incidence of postoperative complications in the recipient rats was 31% (5/16) including 2 cases of peritoneal effusion, 1 case of hemorrhage, 1 case of bile leakage and 1 case of respiratory obstruction. The postoperative 7- and 30-d survival rates of the recipient rats were 81%(13/16)、56%(9/16), respectively. Conclusions The modified technique can establish a stable dual liver transplantation rat model, which deserves widespread application.

ABSTRACT

OBJECTIVE@#To observe the clinical efficacy of adjuvant therapy of moxibustion combined with formula for peritoneal effusion of ovarian malignant tumors.@*METHODS@#Sixty patients with peritoneal effusion of ovarian malignant tumors were randomly divided into an observation group and a control group, 30 cases in each group. The patients in the control group were treated with intravenous infusion of paclitaxel liposome at the first day and cisplatin at the 2nd to 4th day, 21 days as a course of treatment, and 4 courses were given. Based on the treatment of the control group, the patients in the observation group were treated by moxibustion combined with formula. Moxibustion was applied at Mingmen (GV 4), Pishu (BL 20), Shenshu (BL 23), Shenque (CV 8), Guanyuan (CV 4), Zigong (EX-CA 1), Zhongwan (CV 12), Shuifen (CV 9), Qihai (CV 6) and Zhongji (CV 3), each acupoint for 30 min, once a day for 12 weeks. formula was taken twice orally, one dose a day, 2 weeks as a course of treatment, and 6 courses were given. The levels of , , and / in peripheral blood were measured before and after 12-week treatment in the two groups. The changes of peritoneal effusion volume and Karnofsky performance status scale (KPS) score before and after treatment were compared between the two groups, and the clinical efficacy of the two groups was evaluated.@*RESULTS@#The symptom improvement rate was 96.7% (29/30) and overall effective rate was 86.7% (26/30) in the observation group, which were significantly superior to those in the control group [80.0% (24/30) and 56.7% (17/30), 0.05), and the increase in the observation group was significantly higher than that in the control group (<0.05). After treatment, the volume of peritoneal effusion in the two groups was decreased (<0.01), and the reduced volume in the observation group was higher than that in the control group (<0.05). After treatment, KPS scores were increased significantly in both two groups (<0.01), and the increase in the observation group was higher than that in the control group (<0.05). The incidence rate of adverse reactions in the observation group was lower than that in the control group [23.3% (7/30) vs 53.3% (16/30), <0.05].@*CONCLUSION@#Moxibustion combined with formula could effectively improve the immune function, reduce the volume of peritoneal effusion, improve the symptom improvement rate, overall effective rate and the quality of life, and reduce the incidence of adverse reactions in chemotherapy of patients with peritoneal effusion of ovarian malignant tumors.

ABSTRACT

Objective To explore the clinical curative effect and safety of intra -abdominal beacizumab bead sheet resistance combined with hyperthermic intraperitoneal chemotherapy in treatment of ovarian cancer perito-neal effusion.Methods 60 patients with ovarian cancer peritoneal effusion in Binzhou Central Hospital from June 2012 to July 2015 were randomly divided into two groups.30 cases in the control group were treated with intraperito-neal hyperthermic perfusion chemotherapy, while 30 patients in the observation group were given intra-abdominal beacizumab bead sheet resistance combined with hyperthermic intraperitoneal chemotherapy.The short-term effica-cy,toxicity and quality of life were compared.Results The effective rate of the observation group was 73.33%, which was significantly higher than 53.33% in the control group,the difference was significant (χ2 =8.55,P0 .0 5 ) .The improvement rate of life quality of the observation group was 76.67%,which was significantly higher than 50.00%of the control group,the difference was significant (χ2 =8.06,9.31,all P<0.05).Conclusion Intra-abdominal beacizumab bead sheet resistance combined with hyperthermic intraperitoneal chemotherapy has remarkable curative effect in treatment of ovarian cancer peritoneal effusion,with high safety,and can significantly improve the quality of life of patients,that is suitable for clinical application.

ABSTRACT

Objective:To observe the clinical efficacy and side effects of highly agglutinative staphylococcin injection at high dose combined with oxaliplatin or cisplatin with intraperitoneal perfusion in the treatment of malignant peritoneal effusion. Methods:Totally 60 patients with malignant peritoneal effusion were divided into two groups,32 cases in group A,and 28 cases in group B. Group A was treated with staphylococcin at high dose combined with oxaliplatin with intraperitoneal infusion after peritoneal cavity catheter drainage,and group B was treated with staphylococcin at high dose combined with cisplatin with intraperitoneal infusion after peritoneal cavity catheter drainage. All patients underwent the treatment once a week,and continuous four weeks for a courese. The clinical efficacy and adverse drug reactions were compared between the two groups. Results:The complete response rate of group A was 18. 8% ,and the total effective rate was 90. 7% ,which were both significantly higher than those of group B(P 0. 05). Conclusion:Staphylococcin at high dose combined with oxaliplatin with intraperitoneal perfusion is more effective than staphylococcin at high dose combined with cisplatin. Although abdominal pain in group A is obvious,the patients are able to tolerate with it after the symptomatic treatment,therefore,staphylococcin at high dose combined with oxaliplatin with intraperitoneal perfusion can be used as one of effective methods for the treatment of malignant peritoneal effusion.

ABSTRACT

Objective To investigate the value of DNA image cytometry in thediagnose of benign and malignant pleuroperitoneal fluids by comparing with the liquid-based cytological results. Methods There were 417 cases in-volved in this study. Pap stain for cytology analysis and feulgen stain for DNA image cytometry were used to identify benign and malignant pleuroperitoneal fluids respectively,then compared the results of the two methods. Results Clinically or pathologically,213 were classified as malignant and the other 204 as benign. The sensitivity, specifici-ty,accuracy, positive predictive value and negative predictivevalue of DNA image cytometry were 89. 7%,100%, 94. 7%,100%,and 90. 3%,respectively. However,the sensitivity,specificity,accuracy, positive predictive value and negative predictive value of liquid-based cytology were 63. 4%, 81. 9%, 72. 4%, 78. 5%, and 68. 2%, re-spectively. There were significant differences in thesensitivity, specificity,accuracy, positive predictive value and negative predictivevalue. Conclusion DNA image cytometry has great application value in the diagnosis of benign and malignant pleuroperitoneal fluids, and can increase the positive rate, reduce misdiagnosed rate with liquid-based cytology.

ABSTRACT

Objective To investigate the relationship between positive peritoneal exfoliated cancer cells and the clinicopathological features of patients with hepatocellular carcinoma before any invasive treatment.Methods Of the 92 patients with hepatocellular carcinoma who underwent laparotomy,ascites fluid was collected in the patients with peritoneal ascites; and peritoneal lavage fluid was collected in those patients without peritoneal ascites.Then,shedded cancer cells in these fluid samples were detected.Results The positive rates of peritoneal cancer cells were associated with the TNM stage,tumor location and tumor size.The positive detection rate of cancer cells in TNM stage Ⅲ and Ⅳ was significantly higher than stage Ⅰ and Ⅱ (38.1% vs 8.0% ; P =0.0005).The positive detection rate was higher in tumors located closer to the surface (P =0.0 002),and with larger diameter (P =0.00 007).Conclusion Peritoneal cancer cells were significantly correlated with tumor stage,tumor location and size in hepatocellular carcinoma.

ABSTRACT

Objective To discuss the diagnostic value of peritoneal effusion detection by emergency-ultrasound in patients with closed abdominal injury. Method From August 2006 to June 2009,212 patients with closed abdominal injury were studied to evaluate peritoneal effusion detection by emergency ultrasound. Results of 212 patients,peritoneal effusion frequency rate was 78.8%( 167/212), meanwhile,abdominal paracentesis confirmation ratio was only 46.2%(98/212). In the follow-up, 13 patients with injuried hollow viscera and 1 patient with rupture of kidney showed peritoneal effusion. The volume of abdominal fluid was increasing in 17 patients,which needed to be managed by surgery. The accuracy rates were respectively 78.3%( 112/143) and 36.1%(13/36) in the solid organs and the hollow organs. Conclusion During the course of diagnosis and treatment in closed abdominal injury,peritoneal effusion monitoring by ultrasound should be used routinely, which can help to decrease the rate of misdiagnosis and avoid delayed treatment.

ABSTRACT

PURPOSE: It has been reported that in colorectal cancer, the positive rate of the cytological examination of ascites is low and that the cytologically positive result of the cancer cell influences its prognosis; nonetheless, not many studies on the correlation of the formation of peritoneal effusion and cancer have been done yet. Thus, this study on the correlation of clinico-pathological findings with peritoneal effusion was initiated. METHODS: The study population, includes a total of 191 patients who underwent an operation for colon cancer and rectal cancer from May 1, 2004, to December 31, 2005. Peritoneal effusion considered to be present in cases with more than 10 cc of body fluid retained in the Douglas pouch, and a cytological test was performed on patients whose retained fluid was more than 50 cc. In all patients, the correlation of the clinico-pathological findings with peritoneal effusion was analyzed, and the volume of effusion and the positive result of peritoneal cytology were compared. RESULTS: Among the 191 patients, patients without peritoneal effusion numbered 133 (69.6%) and patients with peritoneal effusion numbered 58 (30.4%). Between the two groups, the presence of intestinal obstruction due to cancer (P0.05). CONCLUSIONS: For colorectal cancer patients with peritoneal effusion, but without co-morbid medical diseases inducing such peritoneal effusion, by regarding peritoneal effusion itself as meaningful, the range of lymphadenectomies, adjuvant chemotherapy, and other additional therapy should be considered.

ABSTRACT

Objective To evaluate the efficacy and toxicity of paraplatin used within the cavity for the treatment of malignant serous cavity effusion. Methods Puncturing with catheter of centre vein and permanent catheter, withdrawing proper volume of malignant effusion, 150 ~ 450 mg paraplatin was infused into the serous cavity each time for once or twice a week till the effusion was disappeared or there was no change. Results 76 cases were evaluable for response and toxicity. There were 21 patients with CR, 33 patients with PR and 22 patients with NR. The ORR was 71 %. Among them, the RR in malignant pleural effusion group was 79 %(27/34), 50 %(13/26) in the peritoneal effusion and 87 %(14/16) in the pericardial effusion. Only I ~ II degree bone marrow suppression was observed. Conclusions Paraplatin used within the cavity is more effective and safe for treating malignant serous cavity effusion.

ABSTRACT

There is a small amount of liquid in healthy people's abdomen,but excessive liquid in abdomen has been called peritoneal effusion or ascites.Ascites is a common clinical sign.Though many causes can lead to ascites,cirrhosis,malignancy and tuberculosis are more common.According to different causes and physical and chemical properties,ascites can be divided into extravasate and transudate.It is important to distinguish between benign and malignant ascites,but sometimes the differential diagnosis is difficult.In this paper,the characteristics of the ascites,the procedure of diagnosis and treatment are discussed.
